Portfolio cover sheet
A dated contents page for the portfolio or progress review, so a reviewer can see at a glance what is in it and what period it covers.
Nineteen states require an annual assessment, and in many of them a portfolio or written evaluation is accepted instead of a standardized test — which states, and what each accepts.
A reviewer reads a lot of these. A sheet that says plainly whose work it is, what period it covers and what is enclosed makes the review shorter and friendlier, and it dates the record for you.
